Proofs of Debt and Proxies

Creditors who wish to vote at a meeting of creditors must complete and lodge with our office the following forms:

1. Appointment of Proxy
2. Proof of debt

Creditors are required to complete and return both forms to this office at least 24 hours prior to the forthcoming meeting of creditors in order to be eligible to vote and participate at the meeting.

Attendance to meetings is encouraged but not compulsory.
